Coordinated Care with Your Family Veterinarian

We maintain open communication with your primary care veterinarian from referral through recovery to ensure continuity and excellence in care.

Urgent cases are triaged for timely scheduling. Your veterinarian may reach out directly to prioritize referrals.

Registration & Consultation Process

A registration form is requested prior to your pet’s appointment. Once we receive this form, we will contact your veterinarian for a referral form and medical records:

  • A patient profile is created
  • Our team will contact you to schedule a consultation
  • You may also call us directly, but a registration form is still requested prior to the appointment

Forms Required. Please complete and upload the following forms prior to your appointment:

Surgery Consultation $225.13 (includes tax)

Internal Medicine Consultation and Blood Pressure $282.72 (includes tax)

Dentistry Consultation $225.13 (includes tax)

We prioritize urgent cases

Please inform our reception staff if you require an appointment before the next available consultation.

What to Expect at Your Consultation

How WE Prepare:

  • Create a medical record for your pet
  • Request and attach the referral form from the referring veterinarian and the medical records from all previous veterinary hospitals.
  • The doctor reviews the entire medical history to familiarize themselves with your pet’s overall health and the reason for the referral and creates a summary of pertinent information in preparation for the consultation with your pet.

How YOU Prepare:

  • For internal medicine and surgery appointments: please do not feed breakfast or treats (food fast) the morning of the consultation; water is fine.
  • Administer any anti-anxiety medications as previously prescribed. These may be given the night before and the morning of the consultation or just on the morning of the consultation.

During the Consultation:

  • We discuss your pet’s overall health, lifestyle, and reason for referral
  • A full physical exam (and oral or orthopedic exam, if indicated) is performed
  • A customized treatment plan is developed using a precision medicine approach

We Review:

  • Anesthesia expectations
  • Pre- and post-procedure care instructions
  • Detailed cost estimates
  • Any questions you may have

Procedures may be scheduled for later that day (internal medicine, biopsies, radiographs) or may be scheduled for a future date.
